Become a Patient
Thank you for considering Brownsville Community Health Center as your medical home. In order to ensure that your registration process is completed efficiently, please be aware of the following: Persons with private insurance You will need your insurance card and a picture ID. If you do not have a current insurance card, you can provide a letter from your insurance company that contains information about your medical coverage. Persons with private insurance who wish to take advantage of services not covered by their insurance plan such as dental services, will need to provide proof or income and proof of residency described below. BCHC uses an income sliding fee scale set by the state to determine the percentage amount to be discounted for these additional services not covered by your insurance Persons with Medicaid, CHIP or Medicare Parents of children with Medicaid and CHIP will need to provide a valid picture ID (see below for acceptable forms of identification) and the child’s Medicaid/CHIP card. Persons with Medicaid, CHIP or Medicare who wish to take advantage of services not covered by their insurance plan such as dental services, will need to provide proof or income and proof of residency described below. Persons who do not have insurance (either private or public) and wish to enroll as “self-pay” patients Proof of Identification: For Adults, one identification is required with a picture ID:
Current driver’s License, TX Identification card, Legal Resident documents, Passport,
Voter Registration card, Consulate ID card
If no picture ID bring 2 of the following:
Birth Certificate, Social Security card, voter registration card, school record
Marriage license, Selective Service Card, etc.
For Children:
Birth certificate and social security card (if applicable)
Verification of Birth Facts from Hospital
Proof of income (as applicable)
Earned income
Copy of most recent four paycheck stubs
BCHC form 100 Verification of Employment
Statement from employer w/company’s information
Unearned income
Recent TANF letter from Texas Dept. of Human Services
Social Security Administration and/or Social Security Supplement
Child support award letter from Attorney General
BCHC form 200 Statement of Support w/ID of person writing statement
Retirement Pension award letter, Veteran’s pension letter,
Unemployment award letter from Texas Workforce Commission
Self-employment
BCHC form 500 Self-employment declaration forms/letter last 2 months
1040 Income Tax forms w/W2
Proof of Residency
Utility bill EX: PUB, telephone bill, gas bill etc
Food stamp letter, mail received by applicant
Lease from dwelling (house, apt. Etc.)or BCHC Form 300 from Renter
Eligibility information should be updated every two years or whenever there are changes in household size, household income or if someone in your household obtains or loses medical coverage. If the eligibility information is not updated your account may be charged at the 100% for services until required documentation is provided.
