Thank you for considering Brownsville Community Health Center as your medical home. In order to ensure that your registration process is completed efficiently, please be aware of the following:

Persons with private insurance

You will need your insurance card and a picture ID. If you do not have a current insurance card, you can provide a letter from your insurance company that contains information about your medical coverage. Persons with private insurance who wish to take advantage of services not covered by their insurance plan such as dental services, will need to provide proof or income and proof of residency described below. BCHC uses an income sliding fee scale set by the state to determine the percentage amount to be discounted for these additional services not covered by your insurance

Persons with Medicaid, CHIP or Medicare

Parents of children with Medicaid and CHIP will need to provide a valid picture ID (see below for acceptable forms of identification) and the child’s Medicaid/CHIP card. Persons with Medicaid, CHIP or Medicare who wish to take advantage of services not covered by their insurance plan such as dental services, will need to provide proof or income and proof of residency described below. BCHC uses an income sliding fee scale set by the state to determine the percentage amount to be discounted for these additional services not covered by your insurance.

Persons who do not have insurance (either private or public) and wish to enroll as “self-pay” patients
