Outreach Assistance Worker provides tracking of ordered referrals and establishes communication with the BCHC care teams and patients. The Outreach Assistance Worker consults with supervisor/provider for patient progress and for tracking Pediatric referrals to ensure medical orders are carried out. Conducts community outreach to increase the pediatric caseload, assist with the organization of health fairs, presentations, or community events and other duties as required. High School Graduate and at least one year of college education preferred. One or two years experience working in a medical setting, Pediatric, Adult Medicine OB/Gyn or Family Medicine experience preferred. Experience in planning outreach activities such as health fairs, presentations in a group or one on one. Great communication skills, patient orientated and variation of hours and extended shifts. Organizational skills, and ability to multi task in a fast paced environment. Must follow through and take initiative and work with little supervision. 


Responsible for greeting patients, making appointments, taking payments and handling provider schedules and update patient information in practice management system. Performs some data entry and balance cash payments at the end of the day. Must maintain confidentiality of patient information. Functions as a member of a collaborative health care team to create and maintain a Patient Centered Medical Home. Qualifications include H.S. diploma. Great interpersonal skills and customer oriented. Ability to multi-task and work in a fast pace environment. Ability to work variation of hours and willingness to cross-train. May rotate through the different clinic sites at any given time. Ability to work well under stress and maintain calm under pressure. Six month to one year experience in a medical setting or related field preferred. Available transportation and valid TX driver’s license. 


Duties include weekly tracking of ordered referrals generated by the providers assigned to the Assistant’s care team to meet PCMH requirements. These duties include scheduling pending or cancelled appointments, requesting notes from specialists, entering them into the electronic medical record and registering Social Services patients on a rotating basis. Qualifications include H.S. diploma and formal education as a Medical Assistant. Excellent customer service skills. Compassionate and patient/service oriented. Functions as a member of a collaborative health care team to create and maintain a Patient Centered Medical Home. Strong organizational multi-tasking skills and ability to maturely handle disruptions in a fast paced setting. Computer skills, experience with electronic medical health records, and knowledge of Excel. Professional appearance and attention to detail. Bilingual in order to communicate effectively with providers and patients. One to three years experience in Ob/Gyn, Adult or Pediatrics strongly preferred. Current M.A. Certification or in the process of obtaining certification and CPR certification. From time to time, may be assigned to multiple sites or work variation or extended hours.

**OUTREACH AND ENROLLMENT REPRESENTATIVE – Full-Time (Contingent Upon Continued Funding)

Will provide, education, eligibility and enrollment assistance to uninsured patients of the health center and community residents on the Federal Marketplace Health Insurance Exchange. The Outreach and Enrollment Representative will conduct in-reach, outreach and education activities to existing health center patients and non-health center patients according to federal guidelines about affordable insurance options. Qualifications include High School Diploma or GED required. Degree in Social Work, Education, or health related field. A combination of 2 years experience in Public Health, Outreach or Social Work and college hours equivalent to an Associate’s Degree. Fluent in English and Spanish in order to communicate with patients, the community in general including, insurance carriers and other agencies. Past experience working with public assistance benefit applications preferred. Must have current TX Driver’s License and reliable transportation. Willingness and ability to work evenings and weekends.
