The Health Information Services Department is dedicated to providing prompt processing of all medical record requests while ensuring that all medical information is disclosed in accordance with federal and state laws. All staff will maintain the individual’s right to confidentiality when processing and releasing Protected Health Information (PHI).
Medical records maintained by the clinic are confidential and legal documents that cannot be disclosed to anyone without the consent of the patient with the exception of the required reporting of certain information to state and federal agencies.
The Medical Practice Act, Section 5.08(k) allows a patient to obtain a copy of his or her medical record, or ask that a copy be sent to a new doctor.
A written consent from the patient or the minor patient’s parent or legal guardian must specify the information needed, the purpose for the release, and the name of the person the information is to be released to.

Medical Record Questions
What are the fees for medical records?
Medical record fees are $25.00 for the first 20 pages and .50 cents per page thereafter for personal requests, insurances, etc.
Radiology films $ 8.00 plus mailing fee.
How long do you keep medical records?
Records for adult patients are kept for seven years after the last visit.
Pediatric records are kept until the patient reaches the age21 or seven years after the last visit, whichever is longer.
How long does it take to process my medical record request?
Processing a request may take up to 15 business days after the date of the request and some fees may apply (depending of the type or request). Additional time (up to 30 days) may be taken for unusual circumstances such as staffing shortages, purged records, etc.
Additional Tips:
• Provide a complete consent with all correct information
• If you changed your name, provide both names and a contact number where you can be reached for any questions to avoid delaying the processing.
• No personal information will be given over the phone.
